Below you will find a link to your new patient forms. Please complete these forms prior to your first appointment and bring them with you. This will save you time. If you have questions, please call us.
We suggest that you contact your insurance carrier prior to your first visit to see if they cover naturopathic medicine and acupuncture. CNM Inc. requires payment of all deductibles and / or co-payments, as defined by your primary carrier. Please note that Medicare, Medicaid, and Tricare do not cover our services. If you have questions about your coverage we can help you. Please realize that it is your responsibility to determine your coverage since your insurance company has a contract with you and not us.
WE BILL INSURANCE COMPANIES AS A COURTESY AND WILL WORK YOUR ACCOUNT FOR 90 DAYS. AFTER THAT ALL PAYMENT BECOMES YOUR RESPONSIBILITY.
The providers at the Center for Natural Medicine, Inc. are very busy. It is absolutely necessary for you to give at least 24 hours notice if an appointment needs to be rescheduled or canceled.

Insurance Information
We are a Participating Provider with Blue Cross Blue Shield and Veterans Choice Program. We bill ALL insurances as a courtesy with the exception of Tricare, Medicare and Medicaid as unfortunately they do not cover our services.
